To begin, a travel consent form letter is required for each traveling child. Provide the child’s complete personal information, including name, date of birth, and passport details (this is mandatory if traveling internationally). Additionally, furnish the name and details of the accompanying adult who will be responsible for the child during the trip. Specify the exact travel dates, including departure and return, and if available, flight details.
Ensure you fill in the accommodation details accurately. Include the address and contact information of the primary place where the child will be staying during the travel. In case of multiple accommodations, provide the necessary details for each location separately on another sheet of paper.
For verification purposes, you may want to attach copies of any legal documents establishing custody or guardianship arrangements, such as court orders or custody agreements. When you are able, provide a detailed itinerary of the trip, including flight details, transportation arrangements, and planned activities at each location.
Lastly, ensure the form is signed and dated by the custodial/non-custodial parent or legal guardian who is authorizing the child’s travel. By adhering to these guidelines and providing accurate information, you can help ensure a safe and enjoyable travel experience for you and the child.
